A new horizon for ALS patients
The Community of Madrid is about to take a significant step in the treatment of Amyotrophic Lateral Sclerosis (ALS), a devastating and incurable neurodegenerative disease. In April, a clinical trial will begin that promises to be a milestone in medical research, as it will evaluate the experimental drug AP-2 for the first time in humans. This advancement will take place at the Hospital de La Princesa Health Research Institute, known for its commitment to biomedical innovation.
The study, which has received the green light from the Spanish Agency for Medicines and Health Products, will start its phase with 72 healthy volunteers. The main objective will be to analyse both the safety of the drug and its pharmacokinetics, that is, how it behaves within the body after administration. This type of trial, known as First in Human, is crucial for validating treatments before applying them to affected patients.
ALS affects the motor neurons that control voluntary muscles, leading to a progressive loss of muscle strength and eventual paralysis. Although cognitive abilities usually remain intact, this incurable disease has an average life expectancy of between five and six years from diagnosis. In Spain, between 4,000 and 4,500 people suffer from ALS each year.
The compound AP-2 acts on the TDP-43 protein, whose alteration is associated with the development of ALS. Previous studies have shown that this drug can reverse this alteration and restore the protein balance necessary for the proper functioning of motor neurons. The stepped design of the trial will allow for the administration of progressive doses of the medication, thus ensuring safety before advancing to higher levels.
This trial not only represents an advance for ALS patients; it also positions the Hospital de La Princesa as a leader in early clinical research in Spain. The initiative reinforces the commitment of the Madrid public health system to innovation and therapeutic development against complex diseases.

As this clinical study progresses, the foundations will be laid for future phases where not only safety but also the actual efficacy of AP-2 in halting or even reversing the devastating effects of this disease will be evaluated.
